Under the general supervision of the Team Manager, Staff Services Manager I, in the District Operations Support Team of the Vocational Rehabilitation Employment Division, San Francisco District, the Community Resource Navigator will work closely with Rehabilitation Delivery Team and Community Partners to identify potential resources available, conduct and assist with needs assessment of consumer.
Will collect and record community resource database, maintenance and provide consumer case management, connect consumers with community resource services that lead to success/achieve an Individual Plan Employment (IPE) goal/ Consumer-Centered Human Service Plan.
Community Resource Navigator works cooperatively with the Rehabilitation Delivery Team and Community Partners throughout Marin, San Francisco, and San Mateo Counties in multi-branch office contacts to ensure consistent resource, communication and responsiveness.
The Community Resource Navigator (CRN), Associate Governmental Program Analyst (AGPA), in a lead working capacity provides a variety of high-level analytical support toward connecting applicants, students with disabilities and consumers to community resources available to support their independence and efforts toward achieving an employment outcome.
The CRN navigates consumers to seek community resources available for their IPE needs.
Also, CRN will inform and educate to empower consumers to access community resource services in order to remove employment barriers.
CRN will identify community resources, perform complex analytical duties, and provide subject-matter training toward ensuring the efficacy and consistency of delivering local community resource services, responsiveness to Rehabilitation Service Delivery Teams and the effective delivery of consumer resources across the district.
Working in collaboration/partnership with the counties served within the district, Workforce Development Board (WDB), America’s Job Center of California (AJCCs), and other local county community partners not already working with the AJCC.
In addition, the CRN is responsible for establishing and maintaining contacts with federal, state and local government, and community-based organizations.
