The Care Team Manager will lead an ILS-CA Care Team dedicated to providing high-quality Care Management services to members in a compassionate, effective, timely and professional manner.
The Care Team Manager will be responsible for the monitoring and compliance of the Care Manager’s work.
Other responsibilities include the reporting of Care Manager data, ongoing supervision and support for Care Managers as well as performance improvement.
Essential Functions: Participate in regular case conferences to contribute bring expertise and perspective to the care team, Effectively lead the assigned ILS-Care Team that includes Care Managers, Care Assistants (CA), Community Health Specialists and other staff, ensuring that highest quality services are consistently delivered in a timely and professional manner.
Provide direct supervision and guidance to care team members, supporting them in achieving service objectives, meeting compliance standards and providing excellent customer service.
Manage internal and external relationships across the continuum – ILS CMs, ILS CAs, ILS CHSs, ILS Managers, BSP Members, BSP Providers, ICTs, internal ILS-CA Departments, and community- based organizations.
Ensure that Care Team Members are proficient and fully utilize technological resources and are engaged in staff development opportunities.
Monitor and assess care team performance and modify approach, as needed.
Ensure that Care Team Members are compliant with ILS, federal and state regulations, as well as the ILS Care Management Model(s) of Care Complete administrative tasks including payroll and member assignments, as well as preparation and submission of accurate monthly/periodic reports in a timely manner.
Conduct initial, interim and annual performance reviews in a thorough and timely manner.
Provide ongoing monthly supervision to Care Managers Conduct ongoing team meetings with Care Managers to make sure they are familiar with new protocols and have the support they need to continue to function in their current role.
Conduct ride alongs and provide feedback on the Care Managers interaction with members.
Advocate on behalf of members and heath care professionals Use motivational interviewing and trauma‐informed care practices.
Build and maintain working relationships with hospital staff and other community resources.
Escalate issues to appropriate leadership staff.
Document all services in eCare according to guidelines and workflows Maintain effective and timely communication with Care Team Members, HHP Director and other staff.
Assist in developing and making case presentations for quarterly audits.
Attending training sessions and conferences.
Complete administrative tasks – read/respond to emails, track mileage, maintain ADP, etc.
Effectively the ILS-CA technology platforms and digital communications systems Maintain active license or certification.
Complete other duties as assigned.
Education and Experience Education: RN, MSW, LCSW or Master’s in Psychology Minimum Experience: Five years of health care management experience and experience with Medicare and Medi-Cal populations (preferred).
Knowledge: Competency utilizing the care management technology platform and the Microsoft Office Platform required.
Familiarity with the California managed care environment (preferred) Required Skills Superior clinical skills and perspective Proven management skills Excellent verbal and written communication skills Self-disciplined with excellent analytical and problem-solving skills Ability to work well with others, including staff, clients, members, and the public.
