PRC Baker Places is a non-profit residential treatment agency that has provided the San Francisco community with alternatives to institutional care since 1964.
Our direct care professionals provide counseling and case management services to previously homeless adults living with co-occurring diagnoses of chronic mental illness and substance use disorder.

*Navigation*: Responsible to ensure clients are linked to agency program staff, including housing navigators, to ensure all clients have a documented disposition plan and necessary referrals.
*Intake Primary Duties and Responsibilities* * Collaborates with identified residential and outpatient housing intake staff.
Including staff from: BP residential intake service (MH/SUD programs), Hummingbird programs, Leland House and Co-op outpatient as development of an integrated intake department is completed.
* Works with intake staff and program managers to maintain a high level of client admissions to residential and outpatient housing beds.
* Work with agency Data & Claims and QM Departments to maintain data on units of service, duplicated and unduplicated clients.
Provides monthly report to ACCO and clinical leadership on UOS production.
* Working knowledge of agency contracts and applicable areas regarding required UOS production.
* Reports deficits in UOS production to ACCO and clinical leadership within 48 hours, 2 business days, beyond an identified threshold.
* Ensures that the intake department staff review referral packages for completeness within 24 business hours of receipt.
Collaborates with intake department staff and community referent to remove any barriers to effective referral.
* Develop and maintain Memorandum of Understanding agreements with all referring entities.
* Answers calls from community referent regarding any challenges to a referral during regular business hours.
* Participate in rotating on-call BP emergency schedule.
Maintain a documentation log of calls received, issue/s identified and outcome.
* Maintains monthly communication by phone or email with referring partners to share information about bed availability and to troubleshoot any barriers to referral and admission.
Maintain a documentation log of calls made to which referent, any intake issues identified, and how were issues resolved.
* Prepare and submit to the Associate Chief Clinical Officer a monthly aggregate report of communication between DMHSUDP and community referents focusing on challenges identified and barriers overcome to intake/admission to agency beds.
* Review all _Service or Emotional Support Animal _applications and make approval recommendations to ACCO.
* Complete intake required documentation, if necessary * Coordinates client admissions process with operations and clinical staff.
Develops and maintains professional relationships with referents such as doctors, licensed clinicians, facilities and families.
* Communicates relevant assessment information for appropriate level of care to the treatment team and program managers * Assists outreach efforts as designed/requested by leadership.
* Utilizes supervisory feedback and evaluations to improve performance.
* Other duties as assigned *Navigation Primary Duties and Responsibilities* In collaboration with residential and outpatient program managers ensures all clients in residential and outpatient programs have a documented plan transitioning client from agency services to the community by removing barriers to access and by identifying critical resources for clients, helping them navigate through services and systems, and promoting client health.
* Supervises Housing Navigators.
* Collaborates with identified residential and outpatient housing intake staff.
Including staff from: BP residential intake service (MH/SUD programs), Hummingbird programs, Leland House and Co-op outpatient as development of an integrated intake department is completed.
* Ensure Housing Navigators are assisting clients in completing housing applications, including obtaining all documents necessary for a successful application, in interviews and in grievance processes should applications be denied.
* Ensure clients have necessary resources for moving into new housing.
* Assist tenants with completing any required processes for housing provider’s certification.
* Develop and maintain relationships with the relevant property managers, leasing agents, and/or support services staff at the sites where we are referring our clients for housing.
* Ensure Housing Navigators keep accurate, up-to-date records and documentation in client charts.
DMHSUDP is responsible for documentation compliance in collaboration with program staff and QM Department.
* Submit monthly report to ACCO detailing clients progress towards community living navigation.
Maintain a database with information about who we are working with in the community and for which services.
* Provides referral services as needed.
* Work within and promote the integrated services team model dedicated to harm reduction, recovery, and wellness.
* Achieve a broad understanding of the housing-related resources and services available in the community for clients, particularly those with acute medical and behavioral health needs.
This may include information such as rental assistance programs, IHSS services, food programs, and health and behavioral health services.
* Participate in assigned SFDPH/BHS, MOHCD and other housing service-related meetings.
* Collaborate with outside agencies and teams on behalf of clients to support their efforts to secure housing and resources that are essential to achieving housing stability.
Assist clients in developing and learning appropriate methods of advocating for themselves.
* Develop with Housing Navigators, program staff and clients groups that educate and support clients regarding housing attainment.
* Maintain confidentiality regarding patient health information and comply with all laws of confidentiality and HIPAA requirements.
* Maintain a cooperative and collaborative working relationship with PRC/BP staff and clients.
* Assists outreach efforts as designed/requested by leadership.
* Utilizes supervisory feedback and evaluations to improve performance.
* Other duties as assigned.
*Qualifications:* * Bachelor’s degree in Business, Administration, Social Work, or a related field * 3 years staff supervisory experience, 5 years program/department leadership experience * Qualify as SFDPH-_Electronic Health Record_ (AVATAR or Epic) credentialing competent as an MHRS * Must be able to work flexible hours occasionally, including evenings and weekends.
* Must have effective communication skills and strong intervention skills.
* Ability to adapt to diverse client groups.

* Ability to demonstrate computer literacy, various software programs and technologies competence.
